- Als Community Nurse übernehmen Sie Tätigkeiten im Bereich der Gesundheitsförderung, Prävention, sowie des Krankheits- und Symptommanagements im Rahmen einer niederschwelligen Patient*innen und Angehörigenanlaufstelle
- Sie unterstützen die Förderung der Gesundheitskompetenz älterer Menschen im Rahmen der Patient*innenedukation - spezifisch im Kontext chronischer Erkrankungen - und bieten proaktive Beratungstätigkeiten in allen Lebens- und Erkrankungssituationen an (z. B. mittels präventiver Hausbesuche)
- Sie fungieren als Schnittstelle zwischen den Gemeinden und deren Netzwerkpartner
- Sie arbeiten eng mit Gesundheitsberufen im intra- und extramuralen Bereich zusammen, sind sowohl beratend als auch aufsuchend bei den Menschen zuhause tätig
- Sie entwickeln Projekte wie etwa die Gesunde Gemeinde aktiv weiter
- Registrierung im Gesundheitsberuferegister als Diplomierte(r) Gesundheits- und Krankenpfleger*in
- Mindestens zwei Jahre Berufserfahrung
- Postgraduale Weiterbildung im Anschluss an einen Bachelor in Gesundheits- und Krankenpflege in einem facheinschlägigen Bereich erwünscht oder Bereitschaft, eine postgraduale Weiterbildung im Bereich Family Nursing, Community Nursing oder Public Health Care zu absolvieren
- Vorzugsweise ANP-Absolvent*innen mit konsekutivem Masterstudium
- Kenntnisse der regionalen Versorgungslandschaft
- Freude an einer beratenden, ambulanten Tätigkeit sowie an der Netzwerk- und Schnittstellenarbeit
- Offene, kommunikative Persönlichkeit
- Führerschein der Klasse B
